Restores 25 fixes from v12.3.3 (Issue Blowout 2026) while surgically removing the bearer-token auth mechanism that broke SessionStart context injection. Kept hardening: 5MB body limit, 300 req/min rate limiter, path traversal protection, RestartGuard, WAL checkpoint + journal size limit, FTS5 keyword fallback, idle session eviction, summarize try/catch, session-init health wait, pending-messages cleanup.
